If you've been following me then you know I've been doing some firsthand research on cold exposure over the past few weeks. While there are a number of physiological benefits affording by cold exposure, what honestly got me into it is a desire to figure out why I hate it so much, and see if I can work through it.

One of the things that has been on my radar for close to two years now is taking a cold rinse at the end of showering. I did it for about 3 days one summer, hated it, then never did it again. 🙅🏽‍♂️I've recently resumed said practice, but I'm taking a different, and more logical approach...I'm easing into it.

The same way you wouldn't recommend that a new crossfitter do Murph on day 1, subjecting yourself to ice baths and freezing cold showers first thing in the morning during your initial foray into cold exposure training probably isn't the wisest if you actually want to stick with it.

What I've been doing is simply running my hands and feet under cold-ish water at the end of my showers, and honestly it ain't so bad. Truth be told, I've started to look forward to it as a bit of a pick-me-up. The plan is to progress to colder water, more skin contact, and longer duration...gradually.

My point? Don't feel the need to be the iceman from day one. Ain't no shame in easing in. Training is training is training. Be smart and you'll find that it's a whole more more sustainable.

Been working with my friend @keimcee to improve her overhead position, and I added this drill for her today. Mind you, this was the final thing we did after some manual tx and IASTM to both the cervical and thoracic regions, and some other pattern work. We've also dabbled with some @rocktape applications with good success.

The Cook band (a @perform_better superband can also be used) provides a pattern assist, allowing one to access range that they have the mobility for, but don't yet have the motor control/strength to achieve. The goal is to actively lift the arms with the assistance of the bands NOT just have the band passively pull the arms upward. A contract-relax technique is also utilized, as pulling downward on the band contracts the pecs and lats (muscles which can restrict shoulder flexion), allowing for improved relaxation once the lifting motion is resumed.

This drill can be used for any shoulder flexion variation, be it while holding a dowel, while in child's pose lifting one arm, or while performing the diamond lift off test. Give it a shot and let me know your thoughts. I'll definitely be using this one a lot more.

Do you have tight hips? Are you an athlete? Do you sit down a lot during the day? Constantly in hip flexion? this post is for you 💯

The importance of hip mobility (especially for athletes) is extremely important. It can increase athletic performance *especially explosive movements* at the same time it will reduce lower back pain / help prevent injury. •
If you have tightness in the hip muscles when trying to accomplish regular tasks it could be from lack of mobility (your ability to perform functional movement patterns with no restrictions in your range of motion) Remember flexibility is not the same as mobility. A flexible person for example may not have the core strength or coordination to perform functional movements like a person with good mobility 💯✅🏆

Not just for athletes! Tight hips can generate stiffness, pain and imbalances. If you are not fully functional you may get hip or lower back pain just sitting and walking. Small snippet of me working with my volleyball athlete to help her ankle dorsiflexion. This is an oldie, but goodie ... Love doing this in weightbearing position to because can transfer over to function more easily. 
My left hand here is blocking the talus from moving forward (thus creating a posterior glide of the talus under the tibia as she moves her knee towards me.  We ran through this for 2-3 sets of 10 (or as needed) to achieve what you need. She had a pinch in the front of her ankle with her squat, so this worked out for her nicely.

I did a DM about this a while back, and I think it bears repeating. A diagnosis gives a name, often times just fancy words, to a complaint. It doesn't tell us why that issue is there, or how to fix it. It's basically just a synonym for these symptoms that the patient has stated.

The assessment, now that's where the money is that. That's what's going to tell you why something hurts, and what to do about it. And yes, the assessment absolutely includes the subjective report/history from the patient. It all matters.

When I teach my rocktape courses, I make it a point to explain that we don't have pathology specific taping. That just doesn't make sense. How you tape, (as is the case with any intervention) is going to be based upon the person in front of you, and the assessment that you performed. It's as simple as that.

So, assess, don't guess. Provide a diagnosis for patients as it relates to helping them feel better because they can put a name to their pain/symptoms. But make sure that you both remember that treatment is going to be based off of your findings, not how well you can read a messy prescription.

Sometimes we are limited in the overhead position not by mobility, but by stability/motor control. Passive Range Lift-offs are a nice way to work on gaining control in that end range. The goal is to position yourself in the doorway so that the door jam is at or near your end range of shoulder flexion, this way when you go to work on lifting away from the doorway, you're working on gaining control at end range.

Holding the kettlebell in the front-rack position serves as a nice counterbalance to help maintain an upright torso. The goal is to enter into that deep squat and stay there while working on that end range shoulder flexion.

Keep the torso upright, keep the heels down, and remember to breathe. Sets and reps 🙄 5 sets of 5 trying to hold each for 3-5 seconds.

Improve your overhead stability for the snatch/overhead squat. --- @quinn.henochdpt posted a great drill not too long ago, and so I took it upon myself to post a regression while at the ever dope @crossfithuebner. Shout out to The lovely @naomiwueste831 for the videography skills. --- Sometimes we are limited in the overhead position not by mobility, but by stability/motor control. Passive Range Lift-offs are a nice way to work on gaining control in that end range. The goal is to position yourself in the doorway so that the door jam is at or near your end range of shoulder flexion, this way when you go to work on lifting away from the doorway, you're working on gaining control at end range. --- Holding the kettlebell in the front-rack position serves as a nice counterbalance to help maintain an upright torso. The goal is to enter into that deep squat and stay there while working on that end range shoulder flexion. --- Keep the torso upright, keep the heels down, and remember to breathe. Sets and reps 🙄 5 sets of 5 trying to hold each for 3-5 seconds. --- Like it? Repost it. Don't understand it? Shoulder mobility for Low Bar Squatting

As mentioned above, low bar squatting requires a lot more mobility through your shoulder and thorax, in particular your pecs. Here are 3 stretches you can do for your pecs before low bar squatting.

A -pec stretch in doorway with elbows and shoulder at 90 degrees. Lean 'through' the doorway

B - Band stretches behind the head. Allow the shoulder to rotate outwards and keep your elbow straight

C - dumbell fly isometric holds at bottom. Keep elbows straight and hold for a count of 10s. Use a light weight, not a working weight for flys you would normally use

Had this discussion with one of my favorite patients today, @memali108, and while it's true that at times you might need extra resistance to meet your goals, as long as you know what direction gravity is going, you always have a way to impart some sort of resistance and provide yourself with a challenge.

A whole lot has happened in the year that I've been making these DMs, and while it's tempting to just look at the final product or reminisce about the early days when things were just getting rolling, the truth is, it's the behind-the-scenes, not so glamorous, labor-intensive, what-the-fuck-am-I-doing moments that have gotten me where I am; the messy middle.

I hear love hearing from you guys out there, and I get just as many (if not more) questions about career choices/changes as I do about movement and PT. I think that this DM can serve as a nice reminder to both parties. Whether you're considering leaving your job or finally getting around to rehabbing your back, the advice remains the same: You cannot skip the middle.

The early stages might be scary, but overall you'll likely experience positive changes/gains and it won't be so terrible. The end, if you make it, will see you celebrating your victories. The middle, that place where you've gone too far to turn back but definitely can't yet see the light at the end of the tunnel...that's where the magic happens. That place will suck, but you must go through it. You will make it. I promise.

Solo it's a bit tricky at times so I'm going to make it out to @miami_body_tempering to get some assisted work and really hammer some spots...

ANTERIOR TILT💫 (the insta booty)

Anterior pelvic tilt (APT) is super common, especially amongst the athletic population. We've touched on it briefly before but let's dive a little deeper.

APT describes a forward tipping of the pelvis that results in a hyperlordosis (arch) of the low back: sometimes you can follow this all the way down-stream👇🏻 the femur may roll internally,  causing the knees to fall inward and the feet follow ending up pronated 👣.

Generally muscles that are short/tight are the 👉🏻hip flexors (psoas, illiacus, quads, TFL) and the 👉🏻spinal erectors, muscles that are long/weak are the 👎🏻hammys/glutes and the 👎🏻abdominals.

Several issues with this in the hip region alone: ❌the low back develops a lot of shearing forces with this over arched position irritating everything but mostly that transition from low back to pelvis region, ❌your hip extensors are probs weak so #lowback taking over to initiate extension movement (more irritation) and ❌ if your glutes aren't doing their thang-your hams🐽 and adductors take over, setting you up for upper leg issues🤕.

So how do I interpret it? Talk is cheap. 🤷🏽‍♂️ While theorizing and explaining things about movement is great, you'll never own a movement pattern until you actually MOVE! Similarly, you'll never truly grasp a concept until you DO something. Teach it. Recount it. Make a story out of it. Something. You must bring it to life.

While I do believe that education is the most potent pain mediator that we have, we must not discount the bio portion of the biopsychosocial model, and the fact that true physiological processes are occurring when someone experiences pain, be it at the level of the tissues or the level of the brain. To that end, if we want to create new neural pathways and really tap into nueroplasticity concepts, we must move.

In the absence of proper midline stability/core motor control, forward flexion is interpreted by your brain as a HUGELY provocative and dangerous motion. End result? Your brain says, "oh hell no!" and tightens up your hamstrings via neurological control to prevent you from bending forward and blowing out your back.

When this happens, your tissues have NOT shortened, nor has the physiological makeup of your hamstrings changed. Your nervous system is simply not granting you the freedom to allow those tissues to lengthen because it doesn't trust you!

Take home message: All the stretching and foam rolling in the world isn't going to get you to touch those toes again if you don't work on the motor control that is required for proper forward flexion.
