Your hip flexors are tight, I can almost guarantee it. Sitting for long periods of time, like at work or school allows the hip flexors to become chronically shortened and tight. Hip flexor tightness can lead to low back pain, hip pain and even knee pain. After introducing some basic points on what plantar fasciitis is yesterday, let's address some basic stretches to start to passively release strain or tension. 
Plantar fasciitis appears after a chronic irritation of the arch (or plantar aponeurosis), due to repetitive strain. So, thinking of it as an acute inflammatory injury and applying ultrasound might take quite awhile to actually do anything.... or nothing at all 😳
Instead, we need to get that arch working like a proper bow string again. And that requires the correct connective tissue tension from the ankle and to the foot. Since we walk and stand on our feet all day (well, a portion of my day is not usually 🤸‍♀️😉), protocol should be a bit more movement/active based - of course, depending on each case. 
Yesterday, I showed some early intrinsic foot exercises to get the foot moving. Today, I'll introduce some stretches I like to get things moving properly again to reinforce that bow string - aka - plantar fascia. 
Tag someone who needs some good reminders for feet stretches, and stick around for some more loading exercises in the days to follow! 👊 .

Laying Some Groundwork • Part1

Over the next several days, @michaelbettale (DCs/FRCms) & @suppleperformance are going to be showing some ways to improve foot and ankle mobility/control/function. Our feet are our foundation, so it's important to make sure we function well down there.

Intrinsic foot strengthening exercise. Great for regaining neurological control and awareness of how much your feet can(or can't 🤔) actually move. Nice, slow, and controlled. You may not be able to move with perfect fluidity, control, or independence now, but you can with some practice.

Points of Performance:
Keep your outside 4 toes up and touch your big toe down, and then keep your big toe up while keeping your 4 toes down. Jeg har i mine to siste poster snakket om fysiske faktorer rundt skader/problemer hos idrettutøvere, men som idrettsutøver er du fortsatt et menneske og vi bør absolutt ikke glemme det psykologiske. Vi må også huske på at idrettskader er et ord i seg selv som antyder at noe er skadet, noe det absolutt ikke behøver å være. Smerte kan være et resultat av en hel del faktorer. 
De fleste vet at før man skal tilbake etter en skade bør en rekke, tilpassede og spesifikke fysiske tester rettet mot din idrett fullføres. Testene bør fullføres uten problem for å kunne si at du er klar for å returnere til din idrett. Like viktig er det at de mentalt er klare. Viktigheten av kommunikasjon mellom medisinsk ansvarlig og utøvere er utrolig viktig, like viktig som at de medisinsk ansvarlige prater videre med trenere og ledere. På denne måten kan man legge bedre til rette for utøverne. 
For å kunne vite om de er mentalt klare bør det jevnlig gjennom sesong, under rehabilitering, ved tilbakegang til idrett samt etter en skade utføres psykologisk «screening». Ved å gjøre dette kan man f.eks lette på treningsmengde og press i perioder der utøveren opplever andre stressende livshendelser. Man kan legge rehabiliteringen til rette slik at den er utfordrende og bidrar til å øke motivasjon, humør og selvtillit. Man kan vite om utøveren føler en frykt for ny skade eller prestasjonskrav, og om de har troen på kroppen sin. Og man kan vite om det psykiske etter en skade er tilbake der den var før skaden, slik at det ikke kan være med å prege prestasjon eller deltagelse. 
Det er viktig at vi som medisinsk ansvarlig, behandler idrettsutøvere som mennesker de også, ikke bare som «tøffe» idrettskade diagnoser. Å jobbe med bla. selvtillit, selvbilde, motivasjon, frykt, informasjon og kunnskap vil vi hos idrettsutøvere, på lik linje som mannen i gata, gjøre de bedre rustet. Se hele personen! Plantar fasciitis is such a common diagnosis that many people experience! Though everyone's program will be different dependent on your structure, function and goals, I'm going to run through some exercises/stretches that may be helpful to include in your program this week or even add to your mobility/rest days to prevent it!
🔶 Plantar fasciitis Facts:
🔸Usually presents as pain at the bottom of the heel 🔸 The plantar fascia is not muscle/tendon inflammation, but actually the plantar aponeurosis, which is the connective tissue overlying the intrinsic foot muscles
🔸 It can be caused in people with flat feet AND high arches. If the plantar fascia is too stiff (high arch) or has too much give (over-pronation), there becomes a constant irritation to the fascia. The plantar fascia acts as a bow string. Too tight, or too lose, it looses it's biomechanical purpose for propulsion in gait. .
🔷 Windlass Effect/Mechanism:
🔹 Allows the arch to rise, become tight and compress the foot when the great toe is dorsiflexed - mimicking gait
🔹 Allows the foot to maintain structural and functional structure of the foot. .
🔹 Allows the foot to function as a mobile adapter and a rigid lever 
Try lifting the big toe (or have a friend help you out in standing). Difficult? No arch movement? No dorsiflexion range?! This is a problem!! Start with some of these exercises to start waking things up and get your intrinsic foot muscles working again. As well we go on throughout the week, I'll address other stretches and advanced exercises you can do to keep that foot as healthy as you can!

There are 4 muscles involved in the suboccipital region also called 'Upper Cervical Spine' . . . . . 
1️⃣Rectus capitis post. major
2️⃣Rectus capitis post. minor
3️⃣Obliquus capitis superior
4️⃣Obliquus capitis inferior
. 
These muscles are deeper than all other muscles in the back of the neck and they are very strongly connected to our eyes to give the feedback to the larger neck muscles as to where the head is positioned! The purpose of these muscles is to provide fine motor function in movements of the head.
. 
The actions of trapezius, sternocleidomastoid and other larger muscles that move the head are refined by the relatively small suboccipital muscles. As the neck starts to flex in the lower cervical, our eyes 👀always want to look forward, the head and upper cervical spine starts to extend, the suboccipitals shorten and hold the back of the skull to the top two vertebrae (Atlas and Axis) and lock it in place.
. 
These muscles are also a common area to develop trigger points that can cause terrible headaches as you can see on the red marked zones in the picture. Approximately 50% of the neck rotation should come from the first two vertebrae. The rest of the cervical spine splits the other 50%, gradually rotating less and less as you progress from C3 to C7.
. 
The suboccipital area is very prone to get stiff and immobile, when the upper cervical spine can't rotate properly, that motion get picked up by the lower cervical spine, which we want to be actually more stable!
. 
On the picture you can see a myofascial release technique for the suboccipital muscles as well as a mobilization technique for the upper cervical spine to flexion.

Releasing some of this tissue can directly affect and relieve your headache without the use of any medicine!

We began this procedure with a diagnostic arthroscopy that revealed a loose cartilage fragment within the knee joint. This was removed and we then began dissection to access the lateral femoral condyle where this patient had unfortunately sheered off essentially all of their articular cartilage. Bio-absorbable screws were used to fix the unstable fragment back down to the subchondral bone. The loose cartilage fragment was sized appropriately and fixed in the remaining gap. The procedure was concluded with a medial patellofemoral ligament (MPFL) repair in hopes to reduce the recurrence of dislocation.

Watch how much freer the shoulder feels with elevation. (Warning there will be some soreness during the first couple of movements, but remember no pain,  no gain.) Place the PVC so that it is right where the neck and the shoulder meet.

To the general public.... -if you have to wait 1-2 weeks to get an appointment for Physical Therapy...call another practice....there is a better place out there -if you see a PT on visit #1 and then an assistant or an aide afterwards.....there is a better place out there -if you see a different PT each visit....there is a better place out there -if you just get a heat pack, ultrasound, they stretch your hamstrings followed by ice and electric stimulation....there is a better place out there -if you are told by a PT or MD that you shouldn't list heavy things, workout, run, deadlift, or any other thing you like and want to do......there is a better place out there -if you have failed at one PT facility....don't give up...there is a better place out there that cares about you as a whole person not just a patient with back pain.... Seek out better practitioners and demand more from them because you deserve it.  Copay and deductibles are at all time highs so search for the best.

The primary target of ‘pull-ups’ and ‘chin-ups’ is the latissimus dorsi muscle. The latissimus dorsi muscle (commonly referred to as the ‘lats’) is a large, broad flat muscle on the back. 💪🏾💪🏾 This muscle works to extend, internally rotate and adduct the arm at the shoulder joint.📚 In this video, we have @mannyaina performing wide-grip pull-ups in the first clip, and then close grip chin-ups in the second clip. The key difference between ‘pull-ups’ and ‘chin-ups’ is the increased activation of the biceps when performing close grip chin-ups. 💪🏾💪🏾This is due to the closer, supinated grip, as we know that the biceps brachii muscle is activated by supination (palms facing up). ✊🏾However, both wide grip pull-ups and close grip chin ups will activate both the lats/biceps. Pull-ups are regarded as the best lat-builder, however, if you control the amount of anterior pelvic tilt during the pull-up, it is also an amazing low-ab exercise. 🔑📚Therefore, strengthening the lats can lead to increased deadlift and squat numbers due to this increased core-stability. 🏋🏾🏋🏾It is important to keep the chest up and pull through your elbows when performing pull-ups and chin-ups. Brace the glutes and abs to ensure that you do not swing the knees up for momentum. ❌

It starts with the big toe, working on extension (which we have stressed the importance of). It then goes over splaying all of the toes out. A obesidade é uma epidemia mundial e suas comorbidades matarão milhares de pessoas nos próximos anos.
O balanço energético negativo, seja pela redução do consumo de calorias ou pela prática do exercício físico têm sido visto como recomendação "geral" para o combate a obesidade e manutenção do peso corporal.

Algumas informações importantes no combate a obesidade:
- O exercício físico + dieta tem efeito SUPERIOR na perda de peso quando comparado a dieta sozinho.
- Na comparação dos tipos de ATIVIDADES em intervenções SEM DIETA temos:
🔹Caminhadas (BAIXO efeito, 1 a 2kg em 6 meses);
🔹Atividades aeróbias supervisionadas de 400 a 600kcal por sessão, efeito MODERADO de 4 a 6% de perda de peso em 6 meses, sendo que a intensidade e a frequência parece ter maior influência na perda de peso do que a duração do exercício.
🔹Sugere-se que a mudança do comportamento sedentário (menos do que 5000 passos diários), por pelo menos 10.000 passos por dia, seja uma boa estratégia contra a obesidade, porém, a perda de peso pode chegar a 1% inicialmente.
🔹Treinamento de força: Traz positivas modificações na composição corporal (relação massa magra/massa gorda).
